Inova Health System's Palliative Medicine & Comprehensive Care program is seeking a dedicated Hospice Coordinator to join our dynamic team! The role will be remote or on-site, part-time weekends, flexible. Ideal schedule would be a combination of 2-10s (8a-6p) Fri, Sat, Sun, & Mon.

Hospice Coordinator Job Responsibilities:



  • Explains hospice philosophy, levels of care, goals and services to patients/families, nursing staff and providers to ensure understanding of principles of care.
  • Communicates referrals to hospice agencies as indicated.
  • Maintains relationships with hospice agency staff to facilitate communication and transitions of care.
  • Demonstrates knowledge of community resources to meet patient needs.
  • Provides in-services to providers, nursing staff and Case Management as appropriate. Assesses knowledge of staff and provides information on an individual basis when indicated.
  • Utilizes standards, guidelines, and pathways for care delivery.
  • Identifies trends and develops/implements processes and plans for addressing these trends appropriately.
  • Assists with compliance aspects related to hospice.
  • Collects appropriate outcome and referral data and reports to leaders on a regular basis.
  • Performs other duties as assigned.


Hospice Coordinator Additional Requirements:



  • Work schedule: remote or on-site, part-time weekends, flexible. Ideal schedule would be a combination of 2-10s (8a-6p) Fri, Sat, Sun, & Mon.
  • Education: Bachelor's degree in nursing or social work (BSN or MSW)
  • Experience: Two years of public health/community health, medical/surgical, palliative, or hospice nursing/SW experience. Hospital based nursing/SW experience. Hospice experience is preferred.
  • Certification: If BSN, must be licensed as Registered Nurse in the Commonwealth of Virginia. BLS from the American Heart Association.


Hospice Coordinator Preferred Qualifications:



  • Experience: Hospital based nursing/SW experience in hospice
