Hospice Coordinator Job Responsibilities:
- Explains hospice philosophy, levels of care, goals and services to patients/families, nursing staff and providers to ensure understanding of principles of care.
- Communicates referrals to hospice agencies as indicated.
- Maintains relationships with hospice agency staff to facilitate communication and transitions of care.
- Demonstrates knowledge of community resources to meet patient needs.
- Provides in-services to providers, nursing staff and Case Management as appropriate. Assesses knowledge of staff and provides information on an individual basis when indicated.
- Utilizes standards, guidelines, and pathways for care delivery.
- Identifies trends and develops/implements processes and plans for addressing these trends appropriately.
- Assists with compliance aspects related to hospice.
- Collects appropriate outcome and referral data and reports to leaders on a regular basis.
- Performs other duties as assigned.
Hospice Coordinator Additional Requirements:
- Work schedule: remote or on-site, part-time weekends, flexible. Ideal schedule would be a combination of 2-10s (8a-6p) Fri, Sat, Sun, & Mon.
- Education: Bachelor's degree in nursing or social work (BSN or MSW)
- Experience: Two years of public health/community health, medical/surgical, palliative, or hospice nursing/SW experience. Hospital based nursing/SW experience. Hospice experience is preferred.
- Certification: If BSN, must be licensed as Registered Nurse in the Commonwealth of Virginia. BLS from the American Heart Association.
Hospice Coordinator Preferred Qualifications:
