Overview

The Program Manager oversees implementation of CalAIM services for the Homeless Services Department encompassing Housing and Community Supports and Enhanced Care Management. The Program Manager will track implementation components including, but not limited to, referrals, client enrollment/disenrollment, capacity, services model and workflows, revenue tracking, quality assurance, and overall programmatic and financial sustainability. Supports implementation and possible expansion of CalAIM services to new sites using best practices. Collaborates closely with site-based CalAIM staff and their managers as well as other agency CalAIM managers.

This is a full time, exempt, benefit eligible position in Oakland, CA.

Benefits

Compensation: $72,800 - $76,960/year. We offer excellent benefits including: medical, dental, vision (including dependent and domestic partner coverage), generous leave benefits including ten paid holidays, Flexible Spending Accounts, 403(b) retirement savings plan.

Responsibilities

    Assess and monitor operational and billing workflows to ensure enrollment and revenue maximization
  • Document and educate staff on best practice workflows
  • Monitor referral process, enrollment levels, and utilization rates, and strategizes with other Program Managers to ensure program sustainability
  • In collaboration with data analytics, finance, and billing, develop management reports to support high quality client care, program operations, and financial monitoring to promote sustainability
  • Organize and facilitate monthly meetings on updated performance metrics and other reports tailored to each site where CalAIM services are being implemented
  • Provide training and technical assistance to program management and site staff to support quality of care, utilization, data management, and revenue maximization
  • Develop and submit contract deliverables related to CalAIM services
  • Participate in regular meetings with external partners, such as Alameda County Health Care for the Homeless, Alameda Alliance for Health, and Contra Costa County Health Plan
  • Support expansion of CalAIM services to new homeless services sites and with new MediCal Managed Care Plans that contract with LifeLong
  • Monitor access to CalAIM services across homeless services sites to ensure appropriate staffing capacity and sustainability
  • Monitor CalAIM contracts to ensure compliance with deliverables and collaborate to apply for additional funds as they become available
  • In collaboration with Homeless Services Department administrative and program leadership, identify opportunities for clinical care improvements; through frequent review of analytic reporting, take initiative to improve clinical care through methodical, process-intensive work
  • Promote diversity, equity, inclusion and belonging in support of patients and staff
  • Other duties as assigned

Job Requirements

  • High school diploma/equivalent and completion of a bachelor's degree
  • Minimum of one year of case management experience in a community health or social services agency working with a diverse, low-income population
  • Administrative experience with proficiency in Microsoft Office, Outlook, Word, and Excel

Job Preferences

  • Experience working for a non-profit community health clinic and/or social service organization
  • EPIC experience
  • Lived experience of and/or sensitivity to homelessness, incarceration, foster care, mental health services, substance use services or addiction, or as a close family member of someone who has this experience
