Title: Care Manager (RN)

Location: 75 - 90% remote (Osceola, Orange, Seminole)

Based on tier of member, leadership may ask for them to meet with the member to connect with family, (NOT CLINICAL) more so a supportive liaison

Duration: 3 Months

(FTE Ranges based on experience Minimum is $54,000 - Midpoint if $75,000)

Note: Quarterly meetings held at in office location 6801 W Colonial Drive, Suite E Orlando 32818

Responsibilities:

Required to complete daily outreach calls based on member specific tiers.
Outreach calls are to check in on the members, follow up on their needs, remove any barriers to care for the members.
Based on members needs the call volume will vary 5-20 calls per day
2+ years of clinical nursing experience (pediatric)
