Case Manager Safe Haven IHH (Full Time)

Location: Woonsocket, RI

Safe Haven offers a set of supportive services for individuals who are unhoused and actively using substances, often with co-occurring mental health challenges.

TheIntegrated Health Home(IHH) team is a multidisciplinary team offering psychiatry, nursing, case management, peer recovery support, hospital liaison, and housing support.

The primary focus for this position is care coordination and addressing the social determinants of health with the intention of improving health and well-being. The case manager is integral to this goal, working one on one with clients to remove barriers to care.

The Support includes:


  • Connection to emergency shelter and housing, access to basic needs,
  • Primary care coordination,
  • Access to benefits/entitlements, navigation of legal issues,
  • Building improved natural supports and social environment.

Education/Licensing Requirements:


  • Bachelors degree in social work, psychology, or other related human services field, with at least one year of relevant experience.
  • Must have valid drivers license, reliable transportation, and vehicle insurance.
  • Bilingual applicants are strongly encouraged to apply!
