Description Homebase Medical is getting back to basics. Our mission is to optimize the quality of life for homebound older adults, allowing them to live independently and with dignity in their own homes. We make house calls - enabling access to quality wrap-around services for homebound older adults with complex health needs while improving the quality of their lives, right at home. We offer an array of medical and chronic care services and we're backed by SCAN, a not-for-profit organization that has a history of supporting and improving the health and independence of older adults for over 45 years. Homebase Medical is expanding rapidly into the Tucson, AZ area. We are currently searching for a part-time Advanced Practice Clinician (Nurse Practitioner) to join our expanding house call practice and perform Medicare Annual Wellness Visits. The Part-time Physician Assistant or Nurse Practitioner will work a hybrid of Televideo and in-person home visits. Responsibilities include: Perform Annual Wellness Visits in the home setting Drive to patient locations in your assigned geographic area. Average 7-10 patient visits per day (if working a full 8-hour workday) Provide health education regarding prevention and promotion of health Utilize our EMR system to document patient visit/interaction accurately and timely Knowledge of HCC coding and HEDIS gaps Fluency in Spanish is a plus Other duties as assigned Qualifications and experience: Graduate of an accredited Nurse Practitioner program required Current, unrestricted NP license to practice in Arizona including DEA license Experience with Annual Wellness Visits in the home setting (must live within 15 miles of coverage area) Minimum of 1 year of previous experience utilizing an EMR system with a good working knowledge of HCC coding Digital Retinal Screening experience is a plus Great verbal and written communication skills Current drivers' license, auto insurance and reliable transportation Compassionate and caring attitude with a desire to live our mission, vision and values Tuberculosis Screening Policy To ensure the health and safety of our members, if you are selected for this position, your job offer with Homebase Medical will be contingent on providing proof of Tuberculosis screening upon hire or providing proof of a negative screening within the last year.
