Job Summary

Responsible for the planning, strategy, organization and direction of Prisma Health Home Health, Hospice and Program for All-Inclusive Care for the Elderly (PACE) Health Plans, to include but not limited to, patient care and billing services, customer satisfaction, quality/performance improvement and financial results. Collaborates with other nursing directors, non-nursing professionals, and medical staff leadership to develop and maintain home care and hospice inpatient programs that are responsive to patient needs and are strategically integrated with the mission, goals and objectives of Prisma Health. Ensures compliance with all local, state, federal and JCAHO regulatory agencies.

Essential Functions

  • Develops departmental strategic and operational plans of Home Health, Hospice and PACE within the context of the Prisma Health strategic plan, mission and values.

  • Organizes and deploys resources in a manner that provides services and care in a cost-effective and efficient manner.

  • Develops the annual budget for approval; functions within approved budget parameters, monitoring and justifying variances as appropriate.

  • Ensures all policies, procedures and standards within the scope of the assigned areas meet or exceed the requirements, rules and regulations of state/federal regulatory agencies and accrediting bodies as well as stellar service standards. Serves as Home Health, Hospice and PACE contact for Medicare and Medicaid Fiscal Intermediary for quality and medical necessity denials.

  • Collaborates with other health care agencies and facilities in planning, implementing and evaluating relevant health care delivery systems.

  • Consults and collaborates with other directors, administrators, senior management and medical staff leadership in planning to meet patient needs, system goals and objectives and to ensure continuity and quality of patient care. Mentors, supports and develops managers and other staff in their job functions and responsibilities and professional growth and development.

  • Facilitates and ensures the development, implementation, evaluation and updating of standards of practice for all reporting areas.

  • Plans and leads implementation of new programs for the organization including programs essential to value based purchasing and at-risk contracts. Collaborates with other health care agencies and facilities in planning, implementing and evaluating relevant health care delivery systems.

  • Promotes and encourages collaborative problem-solving, information exchange and program development in areas of responsibility.

  • Serves as Senior Leader for the Home Health Agency, Hospice Program, and PACE Health Plans. Ensures that service lines are marketed internally and externally to a customer base that includes, but is not limited to, physicians, employees, system units and departments, the community and other agencies.

  • Performs other duties as assigned.

Supervisory/Management Responsibility

  • Job has direct and/or indirect supervision of team members that may include final budget authority, hire/termination authority, performance appraisal responsibility and disciplinary authority. Job will be considered a member of management staff at Prisma Health Company or affiliate and will have direct reports.

Minimum Requirements

  • Education - Master's degree in Healthcare

  • Experience - Five (5) years of Multi-facility/Regional Healthcare Business Operations (Home Health and Hospice Preferred)
