The LCSW provides comprehensive clinical social services to adults, children, and families at various locations and specialties within UC Davis Health System's ambulatory care clinics. The LCSW is assigned to the Ambulatory Operations Social Work team. The LCSW will support the Adult Cystic Fibrosis program located at the J Street Respiratory clinic, as well as other clinics within Ambulatory Operations as assigned. The LCSW serves as an integral member of the Department of Pulmonary's Adult Cystic Fibrosis team, providing screening, assessments, short and intermediate-term psychotherapy services, crisis intervention, care coordination, referral to community resources, and other social work services designed to meet the complex needs of patients and families in the clinic setting. The LCSW will be actively involved in providing supportive therapy services and case management/coordination to patients and families seeking services. The LCSW is responsible for assisting with Departmental staffing needs as indicated by working across service lines to complete psychosocial assessments of patients and families to identify emotional, social, cultural, financial, and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation. The LCSW formulates, develops, and implements a comprehensive psychosocial plan of care utilizing appropriate interventions while maintaining the dignity of the patient and family. Department Overview Adult Cystic Fibrosis Clinic is a Cystic Fibrosis Foundation-accredited center. The Adult Cystic Fibrosis program at UC Davis Health, in Sacramento, CA., is a longstanding program established in 1985. The Cystic Fibrosis program is based in the Pulmonary and Critical Care Division. Recognized nationally for clinical excellence in patient care, teaching, research, and service to the community, the Division has over 25 faculty members. The LCSW plays an integral role along with a team of providers, nurses, respiratory therapists, pharmacists, dieticians, and medical assistants supporting adult patients in a clinic setting. The LCSW will provide case management, brief therapeutic interventions, supportive therapy services where applicable, teaching, coaching, and education to adult patients living with Cystic Fibrosis.
