Licensed Clinical Social Worker

The LCSW provides comprehensive clinical social services to adults, children, and families at various locations and specialties within UC Davis Health System's ambulatory care clinics. The LCSW is assigned to the Ambulatory Operations Social Work team. The LCSW will support the Adult Cystic Fibrosis program located at the J Street Respiratory clinic, as well as other clinics within Ambulatory Operations as assigned.

The LCSW serves as an integral member of the Department of Pulmonary's Adult Cystic Fibrosis team, providing screening, assessments, short and intermediate-term psychotherapy services, crisis intervention, care coordination, referral to community resources, and other social work services designed to meet the complex needs of patients and families in the clinic setting. The LCSW will be actively involved in providing supportive therapy services and case management/coordination to patients and families seeking services. The LCSW is responsible for assisting with Departmental staffing needs as indicated by working across service lines to complete psychosocial assessments of patients and families to identify emotional, social, cultural, financial, and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation. The LCSW formulates, develops, and implements a comprehensive psychosocial plan of care utilizing appropriate interventions while maintaining the dignity of the patient and family.

Minimum Qualifications - For full consideration, applicants are encouraged to upload a license and/or certification if required for the position

• Must be an active and unrestricted California Licensed Clinical Social Worker, issued by the California Board of Behavioral Sciences Examiners
• A Master's degree from an accredited School of Social Work
• 2 years of experience in a clinical social service or health care setting (I.e., case management or social work)
• Demonstrated experience performing psychosocial assessments and case management evaluations, including possession of strong interview techniques and ability to conduct Suicide Risk Assessments
• Experience interacting with people of diverse backgrounds and cultures, the ability to identify when differences in culture or language may impact communication, and knowledge of how to resolve those differences appropriately with cultural humility is required
• Demonstrated ability to work with a multidisciplinary team
• Knowledge of community resources and awareness of how to assist patients and families with linkage to appropriate resources
• Demonstrate awareness of and sensitivity to dealings with cultural and socioeconomic diversity.
• Knowledge of child/elder abuse/domestic violence reporting requirements and other significant legislation and laws regulating social work practice, e.g., Tarasoff, patient confidentiality, etc.
• Ability to make appropriate interventions based on law and policy.
• Knowledge and understanding of individual, child, and family development, human behavior and communication, the influence of culture and values in social work practice, psychotherapy, case management, and the relationship between social work practice and medicine
• Knowledge and skills in areas of individual, group, and family therapy
• Solid psychosocial assessment abilities
• Excellent clinical judgment and problem-solving skills
• Knowledge of biopsychosocial and behavioral treatment plans
• Ability to handle demanding workloads involving multiple tasks effectively and efficiently
• Excellent written and verbal communication skills
• Ability to work independently and organize a variety of concurrent activities, set realistic goals and objectives, plan and meet deadlines without compromising quality
• Knowledge of chronic and acute disease and how it influences individual and family functioning
• Basic understanding of computer, Microsoft, and EMR functionality

Preferred Qualifications

• Demonstrated experience working with individuals with Cystic Fibrosis
• Demonstrated recent experience with Electronic Medical Records
• Knowledge and experience with Windows-based computer applications and associated software programs
• Spanish language skills
• Use of Spanish language skills in a professional context
• Knowledge of ACT and other short-term psychotherapy modalities

Department Overview

Adult Cystic Fibrosis Clinic is a Cystic Fibrosis Foundation-accredited center. The Adult Cystic Fibrosis program at UC Davis Health, in Sacramento, CA., is a longstanding program established in 1985. The Cystic Fibrosis program is based in the Pulmonary and Critical Care Division. Recognized nationally for clinical excellence in patient care, teaching, research, and service to the community, the Division has over 25 faculty members. The LCSW plays an integral role along with a team of providers, nurses, respiratory therapists, pharmacists, dieticians, and medical assistants supporting adult patients in a clinic setting. The LCSW will provide case management, brief therapeutic interventions, supportive therapy services where applicable, teaching, coaching, and education to adult patients living with Cystic Fibrosis.
