We are seeking a Lead Case Manager - Young Adult Population to join CTI's Youth Services team, located at the Youth Opportunity Center. The Lead Case Manager will provide advanced levels of support to young adults (ages 14-24) and their growing families entering programming. This includes providing advanced level case management, advocacy, referral information and supporting participants in overcoming barriers towards long-term, independent housing. The Lead Case Manager will supervise and lead members of the Young Adult Population team. Responsibilities Include:
- Training and supervising Young Adult Population Case Management staff
- Receiving, assigning, and monitoring new referrals to the Young Adult Population Programming
- Developing and using network of available services to assist participants and staff
- Supporting and completing onboarding and program entry documents with staff and participants
- Leading and facilitating team meetings to address program and participant goals
- Collaborating with Young Adult Population staff to support participants in service planning
- Establishing and maintaining working relationships with internal/external partners
- Data input, file management, case management software and all administrative tasks as necessary
Qualifications:
- Bachelor's Degree preferred, preferably in a Human Services related field
- Two years of experience working with youth and young adults preferred
- Ability to work comfortably with pregnant and parenting youth in both group and individual settings, preferably with experience advocating for families
- One-year assessment, advocacy and case management work in a Human Service field preferred
- Demonstrated knowledge of community resources available to low-income individuals and families
- Excellent customer service, organizational, oral and written communication skills
- Ability to manage multiple priorities and work in a fast-paced environment
- Proficient computer skills required in basic computer software and data collection systems
- Excellent crisis management skills
- Ability to maintain accurate records and confidentiality of information
- Excellent attention to detail
- Valid Driver's License
