Summary
Responsible for providing administrative support to the case management team, facilitating effective patient care coordination, and ensuring efficient workflow within the department. This role is essential for enhancing communication among healthcare providers, patients, and families. Does this position require Patient Care? Yes Essential Functions -Assist case managers with the coordination of patient care services, including scheduling appointments and follow-ups. -Maintain accurate and organized patient records, ensuring compliance with confidentiality and regulatory standards. -Prepare and compile necessary documentation for case reviews and meetings. -Communicate with patients and their families to provide information on services, resources, and discharge planning. -Collect and input data into electronic health record (EHR) systems to support case management processes. Education Work with an interdisciplinary care team of Emergency Department and Addiction Medicine clinicians to assist with patients' transition to further addiction treatment after discharge; take direction from team medical and clinical leadership as appropriate to optimize patient care. Proactively monitor patients with SUD who enter the Emergency Department to make suggestions to the responding clinician about accessing SUD services like consultation from the Addiction Consult Team or referral to the onsite outpatient Bridge Clinic. Make referrals to the full continuum of addiction treatment services inside and outside the system, including but not limited to outpatient treatment, opioid treatment programs, ATS, and CSS. Maintain an up-to-date understanding of local SUD and related resources and best practices for referral. Provide assistance to individuals/families in accessing a variety of public services that may address barriers to patient follow-up with addiction treatment including financial and medical assistance and directly help to meet needs for food, shelter, transportation, and clothing. Complete follow up phone calls to patients to ensure a successful transition to the treatment organization in the disposition plan. Work as part of a Resource Specialist care team which may include cross coverage in other SUD care settings onsite like the inpatient Addiction Consult Team or the outpatient Bridge Clinic.
