Job Summary Responsible for the management of operations, and staff who serve patients accepted into the Nurse Family Partnership Program (NFP), an evidence-based, nurse home visiting program that improves the health, well-being and self-sufficiency of low-income, first-time parents and their children. Performs a variety of management activities including coordination of staff activities, supervision and evaluation of assigned staff, patient selection, quality assurance and problem resolution. Works within the local community to assure that the NFP Program is implemented in accordance with the established guidelines and contract requirements. Utilizes principles of supervision, reflective practice and staff development to provide primary support and appropriate oversight to Nurse Home Visitors to build and model a strength-based, client-centered culture.
Essential Functions

Performs and monitors a variety of patient care activities to assure the delivery of quality patient care. Activities include home visits; patient care conferences; staff meetings; 1:1 weekly supervision; investigation/resolution of patient care issues; coordination of referrals, admissions, discharges and transfers; staff allocation; direct patient care; staff and interdepartmental coordination. Assign Nurse Home Visitor caseloads and monitor staff's ability to build and manage these caseloads. Performs a variety of personnel activities including the overseeing of recruitment and hiring of all Nurse Home Visitors and administrative data support staff; ensuring that the Nurse Home Visitors and administrative data support staff can implement the NFP philosophy and model; oversee/conducts all Nurse Home Visitor and administrative data support staff employee evaluations, counseling and performance management of assigned staff. Educates, mentors and develops all Nurse Home Visitor and administrative data support staff. Ensures all staff orientation is complete within Prisma Health and NFP Program Guidelines. Performs a variety of quality management activities to ensure that quality expectations and regulatory/accreditation requirements are met. Ensures compliance to Prisma Health, NFP, departmental, safety, environmental and infection control standards. Provides useful developmental feedback, and confronts and resolves issues; provides routine surveillance of potential patient care incidents. Actively participates in the development of program/department budgets in collaboration with the Practice Manager, Office of Philanthropy, Medical Director and First Steps Director; understands and actively participates in monitoring the NFP contract; monitors the ongoing expenditures and ensures compliance with fidelity to the NFP model and contract agreements in collaboration with the Practice Manager, Office of Philanthropy, Medical Director and First Steps Director; ensures appropriate allocation and utilization of program resources. Actively participates in the evaluation of program data in compliance with NFP research and data collection protocols; oversees various short and long-term projects at the direction of the Medical Director and First Steps Director; oversees and ensures timely and accurate data input and utilization of the web-based ETO reports and other reports available through NFP; reads and interprets ETO reports to guide practice; shares ETO reports appropriately with team and partners; evaluates program outcomes and interventions. Performs a variety of customer service activities to ensure that customer service expectations are met or exceeded. Activities include role modeling; staff counseling; field home visits; service recovery and problem resolution; builds the confidence and skills of the Nurse Home Visitors, and administrative data support staff. Ensures that all staff, including the Nurse Manager, attend all required NFP educational sessions as outlined in the NFP pre-training materials. Develops, submits, implements and evaluates performance objectives for self and staff. Is knowledgeable of and adheres to Age-Appropriate Standards of Care for patient population served. Enhances professional growth and development of self and assigned staff through activities such as professional affiliations, educational programs and review of recent literature. Assesses and develops strategies to address the individual knowledge and skill needs of assigned staff. Develops methods for ongoing assessment of assigned staff's developmental needs. Compares available data to structure appropriate continuing education and guidance of staff. Participates in in-service education, workshops, required staff and NFP meetings and research activities. Demonstrates effective communication skills in collaborating with other health team members to ensure optimal functioning of the program/department. Provides adequate orientation and study time to remediate gaps in necessary knowledge and skill. Works collaboratively and proactively on local, state and national issues that may affect sustainability of the program; develop and implement strategies for using local print and electronic media to build community awareness of the NFP in collaboration with GHS Office of Philanthropy, Medical Director, Practice Manager and First Steps Director. Develops and maintains community referral sources for the NFP Program; develops and implements external and internal systems to assure timely receipt and disposition of referrals to and from NFP program. Manages Nurse Home Visitors utilizing the principles and concepts of the parallel process and reflective practice; develops and implements regular practice supervision within the program and conducts field visits, both with the frequency recommended by NFP; effectively assists Nurse Home Visitors in analyzing complex problems and case situations. Performs other duties as assigned.
Supervisory/Management Responsibility
Job has direct and/or indirect supervision of team members that may include final budget authority, hire/termination authority, performance appraisal responsibility and disciplinary authority. Job will be considered a member of management staff at Prisma Health Company or affiliate and will have direct reports.
Minimum Requirements
Education - Bachelor's degree in Nursing. Master's degree in Nursing preferred. Experience - Five (5) years of experience in Nursing. Work experience in public/community health, including home visitation preferred. Maternal/child health services, particularly in community setting preferred. Program management and one-on-one supervisory experience preferred. Work experience with diverse populations, communities and organizations preferred.

Current South Carolina R.N. License (Lawson Code NLRN) A valid driver's license; an acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart; and proof of auto insurance. BLS certification
