The ideal candidate will have both of the following: Must possess a valid driver's license, insurance and have a clean driving record. Own a reliable vehicle. Description We are seeking a Blended Case Manager to join our Adult Behavioral Health team in our Sharon Hill, PA location. Earn $21.83 per hour with benefits for a 37.5 hour week, or $23.83 per hour with the selection of enhanced pay and a scaled back benefits package. Position Details The Blended Case Manager will provide resource coordination, recovery services, and symptom management using a strengths-based model of case management to 20-25 Adults living with a mental illness. The BCM will also provide information and support to families and significant others of the individuals served as well as complete required paperwork, correspondence, and chart documentation. This position requires travel to various schools, homes, and other community locations to provide services and support clients.
