Job Summary

This position is responsible for effectively enhancing the drug therapy outcomes of patients at the Medical Center. This is accomplished through continual assessment of the appropriateness, safety, efficacy and outcomes of pharmacotherapy medication through direct patient care, education to health system personnel and the provision of pharmacotherapy-related outcomes research and performance improvement initiatives.

Essential Functions
  • As a member of an interdisciplinary heath care team, provides safe and effective patient care to a diverse range of patients, including those with multiple co-morbidities, high-risk, high cost medication regimens, and multiple medications following a consistent patient care process.
    * Continual monitoring and assessment of drug usage and expenditures via attendance at medical team rounds. Monitors and motivates the medical staff toward enhancing patient/drug therapy outcomes.
    * Documentation of enhanced outcomes through periodic review of select patient/drug therapy scenarios.
    * Continual assessment of physicians' prescribing habits and appropriateness, safety and efficacy of drug use.
  • Develops and maintains collaborative drug therapy management (CDTM) (ambulatory practice) or "dosing per pharmacy" agreements (inpatient setting) thereby assuming professional responsibility for performing patient assessments, selecting, initiating, monitoring, continuing, and adjusting drug regimens, ordering laboratory tests and patient counseling for selected disease states.
    * Documentation of enhanced outcomes through periodic review of select patient/drug therapy outcomes.
    * Continual monitoring and assessment of the protocol's performance and quality of care.
    * Assessment of the protocol's cost effectiveness.
  • Leverages the data from data resources, including Premier Quality Advisor, Service Line Analytics, and the Epic Business Universe to perform cost-effectiveness and comparative efficacy analyses on high-cost pharmaceuticals and evaluate variation of prescribing patterns of high cost and/or high-risk agents.
    * Aligning the efforts of the Pharmacy Department to Medical Center health-system initiatives through the Center for Pharmacotherapy Research and Quality, the Network Performance Group and Patient and Medication and Patient Safety.
    * Maintaining sustained cost savings through ongoing performance improvement (PI) initiatives aimed to reduce cost through ensuring appropriate utilization.
    * Documentation of completed or ongoing outcomes research or performance improvement initiatives.
  • Demonstrate an ability to manage formulary and medication-use processes, as applicable to the Medical Center.
    * Prepares drug monographs and conducts formulary class reviews for consideration of the P&T Committee.
    * Avoidance of costly formulary medication additions with poor supporting evidence.
    * Re-evaluation of formulary medication additions for outcomes improvement.
  • Provide effective medication and practice-related education to patients, caregivers, health care professionals, residents, fellows and students, and the public through lectures and publications.
    * Documenting the number of educational programs to the medical and nursing staff.
    * Document the number of publications, abstracts or posters annually.
    * Provide annually at least one ACPE-accredited continuing education lecture to the Montefiore HealthSystem pharmacy staff.
  • Precept and advise both Doctor of Pharmacy students PGY1 pharmacy residents during their acute care learning experiences or research/performance improvement projects.
    * Document precepting at least 2-3 pharmacy residents or students each academic year.
    * Evidence of constructive summative evaluations, timely submission of evaluations and feedback from precepted residents or students.
    * Attendance at Residency Advisory Committee or Division of Pharmacotherapy meetings.
  • Involved in the hiring, performance evaluation and development of pharmacists and PGY1 pharmacy residents.
    * Involvement in the recruitment and interviewing of candidate pharmacists.
    * Evidence of completion of annual performance evaluation.
    * When applicable, involved in the peer review process for the Department of Pharmacy.
  • Provides clinical and administrative support to research physicians and the Medical Center's investigational drug research studies.
    * When applicable, will assure the proper preparation and dispensing and appropriateness of investigational drugs used within the institution.
    * Compliance with JC and FDA standards for investigational drugs.
  • Provides assistance to the pharmacy and nursing staff to solve drug therapy dilemmas by acting as a liaison to the medical staff, providing current drug therapy information and delivering of formalized in service training.
    * Assists health care practitioners in remaining current with the management of drug therapy through attendance on medical team rounds, written, formalized consults upon request and participation in educational programs offered to health care personnel.
    *Assures optimal drug therapy outcomes and the avoidance of unnecessary therapies that may lead to toxicity, complications or harm to the patient. This will be accomplished through regular adverse drug reaction (ADR) surveillance and documentation of clinical activities.
  • Demonstrate personal, interpersonal, and teamwork skills critical for effective management and leadership.
    * Ongoing participation in at least one Medical Center medication-related committee.
    * Membership and active involvement in at least one professional organization.
  • Attends routine staff meetings and periodic divisional meetings.
    *Documentation of the participation in departmental meetings and activities to assure effective communication throughout the entire department.
    * To assure that a "team" effort is exerted in identifying and solving problems related to the overall quality of pharmacotherapy, will conduct periodic "problem solving" sessions to improve communication throughout the department.
  • Performs all related management and administrative functions as required by the Division Directors and the Director of Pharmacy.
    * To assure the necessary support for the delivery of pharmacotherapy.
    * To promote and maintain a relationship between the departments of Medicine and Nursing and all other areas of the organization to ensure the pharmacy services are provided to patient uninterrupted. This includes providing staffing and on- call support in times of critical need.
    * Administrative functions are completed appropriately and timely.

Qualifications
  • PhD Completion of a PGY1 Rx Residency Required
  • PhD Completion of a PGY2 Rx Residency Preferred
  • 1-3 years Either completion of a PGY2 residency OR an ASHP-accredited PGY1 Pharmacy Residency with at least 3 years experience in the area of specialization required. Required
  • Working knowledge of Microsoft Office (Word, PowerPoint, Excel), Data Mining software (e.g. Web Intelligence, Clinical Looking Glass), Epic Electronic Heath Record system.
  • If necessary, Remedial work will be done upon hire.
  • Pharmacist Required
  • Completion of a PGY2 Pharmacy Residency Preferred
  • Eligible for Board Certification in Pharmacotherapy or similar certification Required
  • Eligible to be credentialed as an Allied Health Professional Required
