Associates are eligible to apply for a lateral transfer after completion of one (1) year of service in their present position, with the following exception: an Associate who has received a written warning is not eligible for a transfer/promotion prior to six (6) months from date of issuance; an Associate who has been suspended is not eligible for transfer/promotion prior to one (1) year from date of suspension. Part-time Associates who have completed their probationary period can move to full-time vacancies within their department provided such movement will not unreasonably reduce the operational efficiency of that department. Probationary period is three (3) months for full-time employees, and four (4) months for part-time employees who works more than 3/5 of the normal workweek. Probationary period will not apply to shift changes. The LPN receives patients in MAP2 clinic in order to insure the effective delivery of service. This position requires the LPN to work at multiple sites when needed (Derm Center, Scarsdale, Morris Park). The hours for this position are 8am-4:30pm RESPONSIBILITIES: * Provide patient/family teaching and discharge instructions to meet learning/discharged needs as identified by RN. * Able to set priorities, problem solve and use proper judgment in difficult situations and be flexible in non-emergent situations. * Provides and maintains safe environment for patients/families through preventive and interventional measures. * Documents direct patient care as per MMC policies and procedures. * Participates, practices and promotes continuous quality improvement. * Communicates effectively with patients/families and members of the health care team. * Responsible and accountable for following MMC guidelines. * Sterilize instruments and equipment using the autoclave. * Order and maintain medications and supplies to par level for site/s. Ensure exam rooms are appropriately, cleaned and stocked each session and between patients. * Collect and properly label specimens as per MMC policy. Maintain specimen(blood,biopsy) logbooks and report abnormal results to MD. * Greet and direct patients to exam room. REQUIREMENTS: * High School Diploma or equivalent. * LPN Training program and NYS LPN License and current registration to practice as LPN. * Basic Life Support * Effective English communication skills, written, oral and interpersonal. * Ability to use basic data entry and retrieval system. * Additional education required by specialty areas/patient care locations. * Ability to use automated clinical information system, personal computer, telefax, photocopier, telephone, centrifuge, EKG Machine.erilization process and/or Autoclave Machine where applicable. * Misc.
