Director of Community Programs and Partnerships

A strategic leadership position with oversight of the daily operations of a geographically defined Gilchrist neighborhood (e.g. counseling and support, elder medical care and hospice) while ensuring a vision and strategy for future growth. Daily operations would include but not be limited to patient access, operating budget, contractual compliance, knowledge of insurance referrals and preauthorization requirements, health information management, patient care transitions, environmental safety, regulatory compliance, patient safety, quality initiatives, and patient and family experience. This position is responsible for developing and enhancing relationships with health systems, residential care facilities, and community leaders and organizations with a goal of increasing visibility and community affiliations and partnerships. Supervises Neighborhood Leaders.

Education:

  • Bachelor's Degree in Nursing or Health Care Administration or another related field.

  • Master's Degree Preferred

Experience:

10 plus years of progressive leadership experience in a health care related field, preferably in serious illness or end of life care. Demonstrated experience in network collaborations

Patient Population:

Principal Duties and Responsibilities:

  • Oversees the management of the day to day operations of the neighborhood
  • Strategic Planning and Business Development:
  • Develops, implements, and ensures achievement of short and long term strategic goals
  • Maintains physician and community relations to ensure access to care, coordinated care services, care transitions and growth
  • Actively participate in program development
  • Assesses the needs of the community for inclusion in the development of clinical programs and business planning

Financial Management:

  • Manages the budget of designated operational areas
  • Assists with preparation of operating budget and makes recommendations for capital budget in consultation with appropriate leaders
  • Oversight of inventory and analyzes cost of supplies and services
  • Oversees accounts payable issues
  • Processes payroll for assigned areas
  • Accountable for revenue, expenses and overall profitability of assigned areas

Human Resource functions:

  • Manages human resources within established productivity guidelines and the personnel budget
  • Facilitates training and development efforts
  • Oversees and assures completion of employee competencies
  • Monitors and reviews work performance of the neighborhood
  • Assists with interviews and the selection of personnel as appropriate
  • Assures that neighborhood staff have what they need to perform their jobs well

Service Excellence:

  • Monitors, investigates and responds to customer feedback, modifying neighborhood systems as needed in collaboration with quality department as appropriate

Quality and Patient/Family Experience:

  • Directs the clinical practice of the neighborhood (Design and modify activities and workflow to optimize the access, delivery of care and care transitions for all patients and families served by the neighborhoods)
  • Assures compliance with all federal, state, and accrediting body regulations and standards. Works with the Director of Quality when needed to achieve this goal
  • Monitors CAHPS scores and works with neighborhood leaders to develop strategies and action plans to address low performing scores
  • Assist with the develop of performance indicators for all neighborhood services and monitor as appropriate
  • Oversees staff involvement in quality and improvement efforts, general safety, and infection control procedures
  • Utilizes Lean Tools to develop standard work and ongoing improvement initiatives

Pay Range

$86,851.13 - $156,332.04

Final salary offer will be based on the candidate's qualifications, education, experience and alignment with our organizational needs.
