The Pool Physical Therapist practices within the scope of care defined in the Georgia Physical Therapy Act. Plans, implements, and evaluates physical therapy intervention to assigned inpatients and outpatients upon consultation with a physician in order to promote maximal functional efficiency of the patient. Responsible for treatment of all clients appropriately referred to the Physical Therapy Department at Grady and Crestview. Demonstrates an ability to cross-cover a minimum of 3 clinical areas providing Physical Therapy services within the Rehab Therapy Department. Ensures that all written documentation meets the standards set as quality monitors for the department. Maintains knowledge of the hospital and the department's infection control, safety, and risk management policies. Performs an appropriate evaluation of the patient and records it accurately and concisely on the forms provided. Sets objective and quantifiable goals for an individual treatment program that are based on problems identified in the evaluation process, the medical history, the prognosis, and the desires and needs of the patient and their family. Performs all exercise techniques and any other treatment that is indicated by community Physical Therapy standards. Modifies treatment as indicated by patient progress. Contacts physicians and/or makes appropriate referrals to other services. Starts discharge planning at the time of the initial evaluation and plan treatment accordingly. Provides care in a manner that is appropriate for the patient's age; demonstrates knowledge and skills necessary to meet the patient's physical, psychosocial, educational, and safety needs. Designs a treatment program that will enable the patient to meet established goals. Completes initial evaluations, progress notes, discharge summaries, and charges within the electronic medical records according to hospital and departmental guidelines. Required: Master's Degree from a Physical Therapy program that is accredited by the Commission on Accreditation in Education of the American Physical Therapy Association. Required: - A minimum of one (1) years of clinical experience is required. Preferred: - At least one year in a teaching hospital is preferred. Required: All Certifications/Licenses are REQUIRED upon hire unless otherwise stated. -Licensed or eligible for Physical Therapist in the State of Georgia (must secure license by employment start date). -Basic Life Support (BLS) certification. | |
