The Care Partner Center serves as an industry leading model to increase the reliability of wrap around emotional and logistical support exclusively for care partners of patients undergoing care at Sibley Memorial Hospital. This is the first of its kind in Johns Hopkins Medicine and aims to improve quality, safety and person-centered care by creating a resource center 100% dedicate to the patient's care partner.



The Care Partner Center Care Coordinator is a central role in creating a compassionate, responsive, and seamless experience for care partners of patients. This position is a point of contact for care partners seeking

assistance or information to lessen care partner burnout.



As part of the Patient Experience Team, this position collaborates with care partners, physicians, nurses, case coordination, and team members regarding psychosocial needs and community resources in order to improve

patient, care partner, and staff experience.

Shift:

PRN /Days

12 hours weekly

Weekends Required



Education:



  • Master's or other advanced degree such as, but not limited to, medical anthropologist, LCSW, occupational therapy, sociologist, PsyD with experience.


Required Licensure, Certifications, On-Going Training:



  • Trauma informed care certification (obtained within 1st year).



Work Experience:



  • Minimum 5 years' experience working in health care or other related field.


Essential Functions:



  • Responsible for the creation, implementation and development of programs that support care partners within the hospital and in the community, including conducting formal and information educational sessions in conjunction with team member needs. Impact will be measured in improved LOS, employee engagement, wellness and HCAHPS for areas and metrics in scope.
  • Provides guidance to care partners through services, programing, individual and group support based on self-referral or physician/staff referral.
  • Identifies patients with complex discharge planning needs and proactively connects with their care partner.
  • Maintains a current database (and makes it accessible) of care provider resources, including but not limited to lodging, childcare, transportation, and health literacy programs.
  • Overall management and day-to-day operations of the Care Partner Center including scheduling, information and communication, materials, and budget.
  • Facilitate care provider support and focus groups.
  • Recruit and oversee Care Partner Center Emotional Support Volunteers.
  • Maintain current knowledge of care partners and create and implement programs in support of their needs and development.
  • Drive development of care partner programs and services, providing individual and group feedback.
  • Support the overall strategic initiatives of the Patient Experience department.
  • Provide hands-on service for departmental and hospital needs.
  • Manage continuous improvement processes of all programs and services offered to care partners.
  • Documents activities in the medical record.

Salary Range: Minimum 62.00/hour - Maximum 62.00/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
