Apply today to become an integral part of our visionary healthcare team! $5,000 sign-on bonus Job Title: Social Worker Position Summary: Join our dedicated team as a Social Worker, where you will play a vital role in providing exceptional psychosocial and practical care to patients and their families. In collaboration with our interdisciplinary team, you will conduct thorough assessments, identify challenges, set goals, and implement effective care plans. Your efforts will directly impact patient outcomes by developing action plans that address psychosocial barriers to discharge and facilitating complex post-acute care coordination. Essential Functions: As a Social Worker , you will be responsible for: * Patient Screening: Conduct screenings for all patients in your designated area, focusing on those with positive indicators for social work consultation. * Comprehensive Assessments: Complete detailed psychosocial assessments and develop individualized care plans for patients and families requiring intervention. * Direct Care Provision: Offer psychosocial support and transition planning for a full caseload of patients, ensuring their needs are met effectively. * Collaborative Coordination: Work within a multidisciplinary treatment team to facilitate care progression, optimizing patient outcomes through teamwork and communication. * Documentation Standards: Adhere to departmental and regulatory documentation standards by clearly defining problems, goals, and interventions, ensuring timely completion of medical record documentation. * Resource Network: Maintain an updated network of referral resources and actively seek local, regional, and national community resources to meet patient needs. * Interdisciplinary Rounds: Participate in interdisciplinary rounds, effectively articulating the role of social work to your team members. * Flexible Coverage: Be prepared to cover for colleagues during absences and participate in on-call coverage as needed. Qualifications: Education: * Master's Degree in Social Work or Counseling and fully licensed to practice in Maryland required- Both LMSW and LCSW-C will be considered. Knowledge, Skills, and Abilities: * Excellent interpersonal skills and the ability to collaborate effectively within an interdisciplinary team. * Comprehensive knowledge of community resources and how to navigate them. * Strong analytical skills to synthesize data and make informed judgments regarding patient care. * Exceptional oral and written communication abilities to interact professionally with hospital staff, healthcare professionals, patients, and families. * Proven ability to prioritize tasks effectively and efficiently. Salary Range: Minimum 32.40/hour - Maximum 53.48/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
