Description MaineHealth CorporateProfessional - Nonclinical The ATC Case Manager provides direct support to patients and community members by responding to referrals from MaineHealth providers and handling incoming calls from individuals seeking assistance with Social Determinants of Health (SDOH) resources. This role involves conducting timely outreach to patients, assessing their needs, and connecting them with appropriate community programs, benefits, and supports. The Case Manager serves as a key point of coordination between healthcare providers, patients, and social service organizations-ensuring that individuals receive accurate information, smooth referrals, and compassionate guidance. Responsibilities include documenting all interactions, maintaining uptodate knowledge of available resources, and working collaboratively within the ATCL team to improve access to essential services. This is a fulltime position, Monday through Friday, 8:00 a.m. to 4:30 p.m. Summary: Required Minimum Knowledge, Skills, and Abilities (KSAs)
