CJE SeniorLife Weinberg Community for Senior Living is a premier assisted living campus in Deerfield that provides a warm, supportive environment with engaging life enrichment opportunities to enhance the lives of its residents. We help those who require care for rehab, dementia and long-term assistance.
About the Role
CJE SeniorLife Weinberg Community seeks a Social Worker in a Senior Assisted Living Community. This includes brief supportive counseling to residents, case management services to assist residents in safely maintaining their highest level of independence, and general daily support. Participates in a holistic team approach to meet the residents' needs. Services are provided in accordance with the Agency Mission and core values through prescribed procedures and established quality professional care standards.
Essential Duties and Responsibilities include the following. Other duties may be assigned.
* Conducts evaluations of prospective Gidwitz residents as part of the clinical team.
* Conducts initial and ongoing assessments of the resident's emotional, physical, and functional status at Weinberg.
* Assists new residents and their families transition into the community, as well as their transition out during discharge.
* Helps educate families and residents on services available to residents living in Weinberg.
* Sets-up, coordinates needed staff and facilitates care plan meetings for Gidwitz residents.
* May participate in Friend Center and Adult Day care planning as needed. This includes participating in the decision-making process related to client transfers between programs and discharge planning.
* Develops interventions with residents, families, and staff members.
* Provides support to residents and family members as needs arise.
* Provides brief supportive counseling, including grief support to residents and their family members.
* Makes appropriate referrals to other CJE departments as needed.
* Accurately documents resident behaviors and resident/family interactions in a timely manner.
* Co-facilitates a care partner support group.
* Works collaboratively with physicians and other health care providers.
* Communicates with hospitals and rehabs to facilitate transitions back to Weinberg.
* Conducts in-services for staff on various topics related to aging.
* Ensures Service Plans and other needed documentation are done in a timely manner.
* Assists other departments in obtaining important resident related information and documents.
* May be asked to tour prospective or current families at Gidwitz and/or Friend Center.
* Assists with other life enrichment programs as needed (conducting groups/programs).
* Annually completes regulatory requirements for dementia and cognitive defect training in addition to license requirements.
Corporate Compliance
* Complies with safety policies and procedures.
* Adheres to laws regarding the protection, use, disclosure, and release of client and CJE business information.
* Attends required in-services.
* Reports violations of law, regulations, policies, or procedures.
* Reports client abuse or neglect.
* Records documentation timely, accurately, and completely.
* This position has responsibility for HIPAA compliance.
* Exposure to bloodborne pathogens is not likely to occur in this position.
Qualifications

Education and/or Experience
* Master's degree in social work (MSW) or counseling/related field and
* LSW/LCSW or LCP/LCPC preferred.
* 2+ years of experience in healthcare or aging services required.
* Experience and expertise with dementia preferred.
Language Skills
* Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
* Ability to write reports, business correspondence, and procedure manuals.
* Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.
Certificates, Licenses, Registrations
* LCSW preferred.
* LCPC preferred.
Other Qualifications
* Some local travel required.
