Description
Summary: Expedites problem resolution of patient flow barriers through inter-departmental collaboration including direction of environmental services, lab, radiology, pharmacy, case management, respiratory therapy, patient care unit associates and other associates as indicated to maintain patient flow. Maintains communication with the Administrative Supervisor and Patient Intake Center (PIC) throughout the shift. Coordinates patient room assignment, prioritized bed flow issues and communicates plan to PIC. Responsibilities:
- Under the supervision of the Nursing Director of Float Pool, coordinates patient placement activity from one unit to another and reports capacity issues to the Administrative Supervisor and unit leadership.
- Uses systematic processes to facilitate prompt and appropriate patient placement in order to reduce and eliminate barriers within the system which may impede patient flow.
- Participates in multi-disciplinary bed capacity briefing process.
- Rounds through units to identify patients that are discharged and available beds.
- Works closely with unit clinical supervisors and managers to problem solve issues related to admissions, transfers and discharges.

- Initiates capacity crisis interventions when appropriate.
- Completes daily summary of activities capturing flow barriers for future tracking and trending.
- Facilitates monthly reporting of metrics to nursing leadership council.
- Coaches staff to comply with capacity management processes.
- Reviews patient records to identify potential discharges and barriers to discharges. Mediates conflict between departments impacting patient flow.
- Communicates with Director as necessary for patient flow enhancement. Communicates directly with physicians as required to improve patient flow.
- Develops daily communication plan with ED and PACU.
Requirements: Education: High School Diploma. Completion of the Advanced Certified Nursing Assistant training required or must obtain within six months from date of hire or date of transfer. Experience:
- Minimum of three (3) years' experience as a Certified Nursing Assistant in an acute hospital setting, or relevant experience preferred.

Certification, Licenses, or Registrations:
- Certified nursing assistant certification required or must be obtained within 3 months from date of transfer or hire.
- BLS issued through American Heart Association required.
