Job Overview

The Project SEARCH Program Coordinator is responsible for the delivery of direct supported employment services (job coaching) to individuals with intellectual and developmental disabilities involved in the Project SEARCH program at Children's Hospital Colorado.

Responsible for training, coaching and supporting individuals on caseload in their respective roles at Children's Hospital Colorado. The role will work directly with clients to help maintain employment by encouraging communication, flexibility, independence, and professionalism. The individual will serve as a liaison between CHCO and the families/guardians/external support agencies of individuals on caseload. Additionally, this role will support the team member in meeting required annual training and performance management expectations.

Duties & Responsibilities

POPULATION SPECIFIC CARE

  • No direct patient care.

ESSENTIAL FUNCTIONS

An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.

  • Provides direct skills training/job coaching to Project SEARCH graduates employed at Children's Hospital Colorado.

  • Serves as liaison between client, CHCO manager, and client's home support system.

  • Track client progress and success on organization and unit responsibilities - including items like required training, goal tracking and annual mandatory requirements.

  • Support client in accessing and learning CHCO systems related to their employment.

  • Attend new employee organization and track requirements of onboarding process with client.

  • Develops skill building employment plans for work success for each individual client, including the creation and maintenance of job aids and connections to support resources and other tools.

  • Maintains accurate documentation, data collection, reporting and participant progress toward skill development and employment success.

  • Supports successful onboarding of students to CHCO and enrollment in community based programs including CCB, DVR and other partner agencies.

  • Submits accurate billing and supporting documentation of services provided to Community Center Boards (CCB), Medicaid and or Division of Vocational Rehabilitation (DVR) .

  • Participates in program partner meetings, statewide Project SEARCH meetings, and family education sessions as appropriate.

  • Communicates and works as a team with Project SEARCH staff and partner programs.

  • Promotes resources for the ongoing learning, development and well being of team members related to employment success.

  • Attends and participate in annual Service Plan Meetings for clients.

  • Document and report concerns to program leadership, law enforcement and Adult Protective Services as needed.

Minimum Qualifications

  • Degrees
    • High School Graduate. Bachelor's Degree, preferred
  • Area of Study
    • None
  • Experience
    • Required: Two years' experience working in a professional environment, providing direct supported employment services to students or clients. Prior job development experience and two years of experience working with individuals with disabilities.
  • Equivalency
    • An Associate or higher degree, in a related field, may substitute for the experience requirements on a year for year basis.

  • Certification
    • Required to possess active, nationally recognized supported employment certification, or obtain within 6 months of hire.

Salary Information
Pay is dependent on applicant's relevant experience.

Hourly Range: $24.48 to $36.72
