Shifts: M-F, 8 hour days Job Summary The Referral Coordinator LPN at Duke Home Care serves as a key liaison between hospitals, physician offices, and Duke Home Care to ensure the timely and accurate coordination of home health referrals. This hybrid role requires strong clerical and administrative skills, with a heavy emphasis on electronic medical record (EMR) management, chart review, and order entry. MAJOR JOB RESPONSIBILITIES
