Clinical Pharmacist - Hospital (Weekend Only)

$10,000 Sign-On Bonus (Paid at 6, 12, 18, and 24 months)

Position Summary

The Clinical Pharmacist collaborates with members of the healthcare team to deliver safe, effective, and evidence-based medication therapy. This role serves as a key drug information resource for pharmacy, nursing, and medical staff, as well as patients. The Clinical Pharmacist practices in alignment with organizational mission, values, and service standards, while ensuring high-quality patient care and regulatory compliance.

  • Weekend Only Option
  • Three 12-hour shifts: Friday-Sunday
Key Responsibilities Clinical Practice
  • Evaluate medication orders and patient records to ensure appropriateness of therapy.
  • Assess patient conditions and develop individualized medication care plans that minimize adverse effects.
  • Monitor drug therapies through chart review and interdisciplinary collaboration; intervene when necessary.
  • Consult with physicians, nurses, and patients regarding medication selection, dosing, administration, interactions, and side effects.
  • Document medication plans, interventions, and patient outcomes accurately.
  • Provide recommendations on drug therapy, including dose, route, schedule, contraindications, and potential interactions.
  • Serve as a clinical resource with developing or maintaining subspecialty expertise.
Collaboration & Patient Care
  • Partner with healthcare teams to ensure optimal patient outcomes.
  • Maintain positive working relationships with medical and nursing staff.
  • Provide education and guidance to patients and caregivers on medication use.
Quality, Safety & Compliance
  • Adhere to all confidentiality, safety, and risk management policies.
  • Participate in quality improvement initiatives, including medication use evaluations and adverse drug reaction reporting.
  • Ensure compliance with regulatory, licensing, and accreditation standards.
  • Complete required inspections (e.g., expiration dating, floor stock checks) and safety training.
Education & Professional Development
  • Participate in precepting pharmacy students and residents.
  • Support continuing education through presentations, in-services, and staff training.
  • Contribute to competency development for pharmacy team members.
  • Attend conferences, workshops, and maintain licensure requirements.
Operations & Coordination
  • Coordinate activities with department staff to ensure efficient workflow.
  • Exercise sound judgment in prioritizing tasks and managing resources.
  • Participate in departmental initiatives, committees, and policy reviews.
Technology & Systems
  • Utilize pharmacy information systems for order verification and clinical documentation (e.g., Epic).
  • Access and apply drug information resources (e.g., Micromedex).
  • Operate pharmacy automation systems (e.g., Omnicell, McKesson, ScriptPro) as needed.
Professionalism
  • Demonstrate exceptional communication and customer service aligned with service standards.
  • Maintain professionalism, teamwork, and respect in all interactions.
  • Adhere to dress code, hygiene standards, and PPE requirements.
  • Effectively manage workload and meet deadlines in a fast-paced environment.
Minimum Qualifications Education
  • Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy
Licensure
  • Current pharmacist license in the state of practice
