Work Type: Full Time (80 hours bi-weekly) (Flexible scheduling to meet patient needs)

Hybrid role: Visiting patients. Office for meetings when needed.

Shift: Days, 8:00:00AM to 4:30:00PM, Monday - Friday, Flexible to meet patients and business needs

Minimum to Midpoint Pay Rate: $27.57 - $35.84 / hour

This is a driving role.

Covers the Lee County area (shared by two MSWs)

Compassionate and patient-focused Medical Social Worker needed to support the adult population in the Home Health setting.

The Medical Social Worker will assess patients psychosocial needs, provide emotional support, assist with community resource coordination, and collaborate closely with the interdisciplinary team to promote safe and effective care in the home environment.

This role requires strong communication skills, critical thinking, empathy, and the ability to advocate for patients while supporting their overall well-being, independence, and care goals.

Education: Masters Degree in Social Work required.

Experience: Minimum of one (1) year of social work experience in medical or healthcare setting.
