The Pharmacy Technician Discharge Coordinator I ensuries that patients receive their prescribed medications before leaving the hospital. The Discharge Coordinator collaborates with providers, extended care team members, pharmacy staff, and insurance companies to triage discharge prescription issues and facilitate medication access for patients. They maintain accurate medication records to pre ent errors and ensure patient safety. They prepare discharge medications by compounding, packaging, and labeling pharmaceuticals. Activities include discharge prescription data entry, adjudication, and delivery and obtaining and organizing information such as patient demographics, insurance information, medication lists, education needs, and information for a pharmacist to determine the appropriateness of discharge prescriptions. The Discharge Coordinator promotes the safe and effective use of medications by providing the pharmacist with accurate and timely information necessary to make informed clinical recommendations. The Discharge Coordinator participates in performance improvement activities to increase the scope and quality of pharmacy services. The Discharge Coordinator performs other duties of a pharmacy technician, including but not limited to preparation and packaging of medications, inventory maintenance, and appropriate medication documentation, as required. Shift: Rotating Shift Minimum Qualifications:
Salary Range: Minimum $20.53/hour - Maximum $33.90/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
