Position Overview
The Licensed Master Social Worker (LMSW) is responsible for conducting psychosocial assessments, providing supportive therapy, and coordinating discharge planning for patients in an inpatient healthcare setting. This role involves collaborating with interdisciplinary care teams to support patient care plans, identify psychosocial needs, and facilitate appropriate referrals and post-discharge services. The ideal candidate will have experience working in hospital-based inpatient settings, including medicine, surgery, or rehabilitation units.

Job Responsibilities




  • Conduct comprehensive psychosocial assessments for patients and families



  • Provide supportive counseling and therapeutic interventions as appropriate



  • Develop and coordinate discharge plans to ensure continuity of care



  • Facilitate referrals to community resources, rehabilitation programs, and support services



  • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams



  • Maintain accurate and timely patient documentation in accordance with organizational and regulatory standards



  • Advocate for patient needs and assist with care coordination throughout hospitalization



  • Participate in treatment planning and patient care discussions




Job Requirements




  • Valid Licensed Master of Social Work (LMSW) license issued by NYSED



  • Minimum of one (1) year of LMSW experience required



  • Prior hospital or inpatient healthcare experience required



  • Experience in inpatient medicine, surgery, or rehabilitation settings preferred



  • Strong interpersonal, communication, and organizational skills



  • Ability to manage multiple priorities in a fast-paced healthcare environment



  • Knowledge of discharge planning processes and community resource coordination




Education




  • Master's Degree in Social Work required




Work Experience




  • Minimum of one (1) year of experience as a Licensed Master Social Worker required



  • Hospital-based inpatient experience strongly preferred
