Responsibilities

The purpose of this position is to answer all incoming intake calls and provide appropriate resolution. Conducts initial evaluations to determine an appropriate treatment level of care. Responsible for completing admission paperwork, including admission data entry. This position requires a person to work with minimal supervision. This position will work with all hospital units, business office, medical staff, medical records, utilization review, marketing, nursing, and data processing.

Essential Job Fuctions are:

Performs telephonic risk assessment to determine level of suicidality/homicidality with any caller in crisis.

Completes call forms and obtains accurate and necessary information

Uses therapeutic and crisis intervention skills to obtain resolution for the caller

Documents all call outcomes/referral information in the call system

Communicates possible admission/transfer information to co-workers

Explains the evaluation and admission process to the potential patient/referral source and informs patient of necessary items to bring for hospital stay

Makes certain that all patient assessments are conducted professionally and that all necessary information is documented

Demonstrates a working knowledge of DSM-V criteria for subsequent diagnosis and referral. Demonstrates basic familiarity and knowledge of major diagnostic criteria for children, adolescents, adults and geriatric population.

Gathers all information pertaining to patient's physical, psychological and social status

Obtains all necessary information on patient's registration form

Completes Patient Observation form on all perspective patients during level of care assessment until patient is safely on unit or until after physician consultation for another level of care.

Obtains all financial information relative to establishment of financial responsibility

Pre-authorizes all admissions and clearly documents the outcome of the authorization/communication

Documents assessment outcomes on appropriate forms. All documentation is clear, concise and timely. Ultilizes appropriate terminology in documentation

Demonstrates effective professional judgment

Demonstrates professional behavior and interpersonal skills reflective of the faciliy mission statement and philosophy.

Exhibits excellent customer relations skills as evidenced by supportive and constructive communication with all contacts including coworkers, patients, visitors, families and referral sources.

Assimilates assessment information into correct disposition/ level of care

Consults with next MD on rotation for consultation

Recommends appropriate level of care with no more than two (2) errors per year. (An error is defined as recommending a lower level of care than what is needed per clinical information.)

Receives and records all reservations for beds and maintains a continuing record of bed utilization

Follows EMTALA guidelines on incoming admissions and transfers

Accurately enters all admissions in the computer and documents all admissions on the appropriate forms

Understands job duties of all shifts including overnight and weekend shifts. Receives and gives thorough report from and to other shift staff.

Communicates information to the staff relieving them from their shift including all planned admissions and activity

Completes all admission paperwork when a patient is admitted to a program, including taking patient photos

Obtains all necessary consents and signatures within hospital guidelines.

Completes admission paperwork and distributes the paperwork to the appropriate departments.

Conducts notification phone calls upon admission or assessment disposition to referral sources

Documents referral information in the call system

Reviews all admission paperwork ensuring thoroughness and accuracy.

Recognizes own limitations, seeks information and advice when unable to solve conflicts/problems on an individual basis.

Is receptive to supervisory feedback and appropriately utilizes feedback. Accepts other assignments and changes in duties or special project assignments with professionalism.

Demonstrates understanding of Zoom software

Demonstrates procedure for contacting technical support

Demonstrates operation of teleassessment equipment and maintenance of environment

Identifies procedures for coordinating transportation to The Brook Hospital for admission to facility

Follows procedures for all MIW & Involuntary admissions

Reports alleged abuse to proper agencies (CPS/APS)


Qualifications

This position requires a minimum of a Master's Degree in a behavioral science field, advanced certification preferred.
