Position Details:



  • Assist in providing rehabilitation and support services for individuals with mental health and behavioral health diagnoses.
  • Support treatment plans, including substance abuse services, education, and family consultation, under clinical supervision.
  • Provide crisis intervention services as needed.
  • Collaborate with families to offer education, support, and guidance.
  • Collaborate with various community resources, including healthcare providers, social services, housing agencies, and other support organizations, to ensure comprehensive care for clients.
  • Ensure the safety and well-being of consumers in residential programs by adhering to safety protocols.
  • Participate in direct care responsibilities, including daily living support and therapeutic activities.
  • Follow and contribute to the treatment planning process in collaboration with the clinical team.


Assertive Community Treatment (ACT) Teams are composed of professionals, including psychiatrists, nurses, mental health professionals, substance abuse professionals, employment specialists, housing specialists, and a peer specialist, all of whom collaborate to provide complete services, including therapy, medication management, and resource linkage to adult individuals with serious mental illness (SMI). Services are provided in the individual's home or the community and include at least three home visits a week. Will berequiredtoutilizepersonal vehicles for client transportation and community-based work.

  • Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or another behavioral scienceandexperience working with adults with serious mental illness (SMI);or
  • Bachelor's degree in a non-behavioral science field with experience supporting adults with serious mental illness or individuals with similar human service needs;or
  • High school diploma or equivalent with relevant experience working with adults with serious mental illness or individuals with similar human service needs.
  • Related training or certifications (e.g., Certified Occupational Therapy Assistant, Home Health Aide) are preferred.
  • Experience in human services, healthcare, education, or other helping professions is a plus.
  • Strong interpersonal, communication, and relationship-building skills.
  • Ability to provide rehabilitation, recovery-oriented support, and community-based services as part of a multidisciplinary team.
  • Valid PA Driver's License.Travel is necessary to offer support across Beaver County.
