Optum Home & Community Care is seeking a Nurse Practitioner or Physician Assistant to provide comprehensive primary care services to patients in their homes. This role focuses on delivering longitudinal, relationship-based care to medically complex patients while working closely with an interdisciplinary care team to improve outcomes and quality of life.

Required Qualifications
  • Current NP or PA licensure and board certification
  • Active, unrestricted state license (or ability to obtain)
  • Active DEA license or ability to obtain
  • Active driver's license and reliable transportation
  • Ability to conduct home visits and travel throughout the assigned territory
  • Experience caring for adult, geriatric, or medically complex patients

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
