DESCRIPTION

The Pharmacist II will be responsible for providing direct patient care to patients in various populations at UCSD Health through the transitions of care (TOC) model. The pharmacist will work in collaboration with providers, perform various TOC activity (medication reconciliation, counsel patients upon discharge, conduct post discharge follow-ups), and may conduct clinical visits with patients, and other duties as assigned. As a float pharmacist, this position will provide coverage across the existing services within cardiology, heart failure, and medicine patients, ensuring continuity of pharmacist-led services during staffing fluctuations and evolving service line needs. In addition, the Pharmacist II will also serve as a preceptor to pharmacy residents and students as assigned and will participate in Medical Center and Departmental committees and promote institution-wide medication use policy. The pharmacist will collaborate closely with physicians, advanced practice providers, nurses, case managers, social workers, and other interdisciplinary team members to improve medication-related outcomes and reduce preventable readmissions. The pharmacist may work under Collaborative Practice Agreements to optimize pharmacotherapy, provide disease-state management, improve medication adherence, and achieve therapeutic goals while ensuring safe and cost-effective medication use.

Responsibilities include (but not limited to):

  • Participating in multidisciplinary inpatient rounds and care transitions planning for high-risk patients as needed to assist with covering for clinical gaps
  • Performing comprehensive admission and discharge medication reconciliation.
  • Identifying, resolving, and documenting medication-related problems.
  • Providing discharge medication counseling and patient education utilizing teach-back methodologies.
  • Conducting post-discharge follow-up calls and clinical assessments to identify adherence barriers, adverse effects, medication access issues, and care coordination needs.
  • Facilitating medication access, prior authorizations, appeals, financial assistance programs, and discharge prescription coordination.

The successful incumbent will be a self-starter who is accountable and requires minimal direction and supervision. This individual must demonstrate adaptability and the ability to rapidly transition between multiple clinical environments, patient populations, and workflows while maintaining high-quality patient care and operational efficiency. The incumbent must be open to new ideas and comfortable working within a large, complex academic health system. The pharmacist will demonstrate professionalism, credibility, commitment to excellence, innovation, sound judgment, and accountability. Given the float nature of the role, the pharmacist must be able to effectively prioritize workload, independently assess service needs, and maintain clinical competency across multiple medicine and cardiology practice settings. The position requires flexibility to work variable shifts, including weekends, holidays as necessary.

The pharmacist must demonstrate knowledge of state and federal pharmacy regulations, clinical pharmacy practice standards, and transitions of care best practices. Strong interpersonal, customer service, teaching, writing, and organizational skills are required. The pharmacist must communicate effectively with patients, caregivers, providers, and healthcare leaders, translating complex clinical information into actionable recommendations. Additional responsibilities may include supporting pharmacy residency training programs, participating in departmental research and quality improvement projects, developing clinical protocols, presenting educational programs, and contributing to departmental strategic initiatives related to medication safety, patient throughput, readmission reduction, and value-based care.

MINIMUM QUALIFICATIONS
  • Doctor of Pharmacy degree (Pharm D) or equivalent training.

  • Registered Pharmacist in the State of California.

  • Meet the minimum experience qualifications for this position in 1 of 3 ways:1) possesses a PGY 2 or 2) possesses a PGY 1 plus 1 year of pharmacist experience, or 3) possesses 4 years of pharmacist experience.

PREFERRED QUALIFICATIONS
  • Completed residency in transitions of care or internal medicine or cardiology.

  • Four (4+) or more years of experience.

  • Experience in transitions of care/discharge models.

  • Experience with working a variety of patient populations (including but not limited to internal medicine patients, cardiology patients).

SPECIAL CONDITIONS
  • Must be able to work various hours and locations based on business needs.

Pay Transparency Act

Annual Full Pay Range: $200,010 - $238,972 (will be prorated if the appointment percentage is less than 100%)

Hourly Equivalent: $95.79 - $114.45

Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable).
